These Membership Transfer Forms will facilitate the transferring of a member from one local church to another local church. Use the forms to transfer a member from one local church to another. The local church clerk must fill out the forms after the church conference has granted a membership transfer. Each form should be sent to the receiving church as a notice of membership transfer and must be accepted by the receiving church's conference before a transfer is complete.
